List values on custom profile form

Hello,

I’ve a couple of lists and I want to get their values in my custom profile form. The list should be only visible on the subscription page.

But when I save the profile page, the value of the list is gone from the subscribers information.
Is there anyway I can the check/uncheck list on the profile page?
