How do I add an unsubscribe link to a newsletter template created with PHP?

I can’t figure it out using the available documentation and Google wasn’t much of a help either.

So I created a newsletter template with PHP/html and I want to add a unsubscribe URL at the bottom of the template, what is the correct code to use for the unsubscribe URL in a PHP form?
