Creating a form to collect information

Hello

I am trying to create a registration form to use that collects the following information with field names in two languages (EN & AR):

Email address
First name
Last name
Company Name
Job title
Phone number

It this the right tool? if yes, could you please explain to me how to do it.

If someone can point me at the right direction, it is going to be really helpful. I have tried so many things and couldn’t get it to work.
Join our newsletter filed is not enough for me. I need a way that visitors can record more than an email address.
